It is your responsibility to read and understand the terms of this contract. If you are not in agreement with the terms, you must address these concerns with your adoption counselor at this time.Your signature at the bottom indicates that you have read, understood, and agree with each statement.

1. SAFPR believes the animal you are adopting to be in good health, unless otherwise stated in the contract. However, SAFPR makes no guarantees as to the health of the animal. SAFPR has each animal checked by our veterinarian prior to coming to our facility and all necessary shorts including rabies, spay and neutering (except when the dog is too young), worming, nail trimming, and flea treatments have been performed. Also, SAFPR requires that your new pet receive microchip identification.

2. You agree to have the dog examined by a licensed veterinarian within 60 days of adoption.

3. SAFPR requires you to contact us if there is any change to your living conditions which would make it a hardship to keep the adopted animal.

4. You assume responsibility for this animal’s entire lifetime. You are responsible for regular and emergency medical treatment, future vaccinations, licensing, food and care of the animal. Should any behavior problems arise, you are responsible for providing proper and humane behavior training. In the case of irrevocable behavior problems SAFPR should be contacted. SAFPR will consult with you to identify and assist with the corrective action.

5. If at any time, for any reason the adopter is unable to keep the above-mentioned animal, the adopter agrees to return the animal to a sister rescue of SAFPR's choice. The adopter will transport the animal back at their own expense to the sister rescue, if this becomes necessary. The animal may never be transferred, taken to another shelter, sold or otherwise given away without the written approval of SAFPR. In addition, you agree that you may not euthanize the dog except in the case of the dog’s terminal illness, injury, or old age accompanied by pain and suffering. The euthanasia must be performed by a licensed veterinarian in a private clinic or hospital.

6. You agree to make a serious effort to find the dog if it becomes lost, by immediately (a) filling lost reports with the local police, animal control authorities, animal shelters, SPCAs (Society for the Prevention of Cruelty to Animals) and local veterinarians and (b) posting lost dog signs and (c) contacting SAFPR immediately should the dog become lost.

7. Any dog you adopt from SAFPR will NOT be chained to a tether. The dog may be contained by an outside run, but only part of the time, NEVER on a permanent basis. Any dog adopted from SAFPR will NOT be transported in the back of an open truck, or similar vehicle. In addition, any dog adopted from SAFPR will NEVER be allowed to roam free unsupervised.

8. You hereby fully and completely release SAFPR and its officers, volunteers and agents from any claim of action, damage, losses, costs, expenses, loss, and cause of action or liability for any illness, your other animals may develop, even if those illnesses are procured from the animal adopted from SAFPR.

9. No refunds or exchanges will be provided. However, a full refund will be provided within 30 days of adoption if the animal poses a threat to another animal or person(s). Should that be the case, the animal will be returned to SAFPR and the petition for refund will be subject to review by SAFPR management and (if necessary) a behaviorist.

10. SAFPR believes the animal you are adopting to be of sound temperament. Please note that any animal has the potential to bite or cause injury and you are responsible for any injury caused by the animal to yourself or others. SAFPR has provided cursory health checks and disposition evaluation. SAFPR makes no guarantee whatsoever as to the mental disposition or training of the animal. Please note that an animal needs some time to adjust to new surroundings. SAFPR recommends that you watch the animal closely and make certain it is not left alone with cats, other animals, children or any strangers, for at least 3 weeks. It is your responsibility to provide a safe and humane environment for your newly adopted pet while they are settling into their new home.

11. You agree to never use the dog for dog fighting or in any other “sport” in which one animal is pitted against another, and to never beat or taunt the dog in order to promote aggressive characteristics.

12. In the event that we make the determination that you have failed to comply with any of the terms of this contract, or if the dog is abused, neglected or in danger, as determined in your sole discretion, one of our remedies will be to immediately recover the dog. Upon demand, you will immediately and without hesitation, surrender the dog to SAFPR. In addition, without waiving any of our rights under this contract. If you do not immediately surrender the dog to us, you will post a payment in the amount of $5,000.00 with SAFPR. This amount is agreed between us as just and reasonable liquidated damages to compensate SAFPR for our costs and expenses, including legal fees, court costs and attorney fees in connection with your breach of this contract and our subsequent enforcement thereof.

13. Neither SAFPR, its volunteers, or its officers are liable to you for any claims, legal actions, losses, injuries, damages, costs, expenses, or liabilities whatsoever in connection with your adoption or ownership of the dog.

14. SAFPR believes any animal deserves good nutrition. Please, take the time to reach out to your adoption counselor in regards to the best way to feed, treat, and care for your dog’s care and wellbeing. We can help you find high quality food, and treat it that fits your schedule and budget.

15. At the time of adoption, we believe the animal to be in good health, with the exception of anything noted on the medical records, but no guarantees are made. It is recommended that all dogs be re-tested for heartworm in 6 months and cats re-tested for FIV/FELV in 6 months. It is also suggested that all dogs be kept on heartworm prevention until the next negative test and then as recommended by your vet.The incubation period for heartworm is 6 months. Dogs who have tested negative or are too young to test should be tested in 6 months. Until that time, they must remain on Advantage multi or an ivermectin based heartworm prevention. If your adopted dog tests positive at that time, the adopter understands that the dog will require treatment at the adopter's expense and SAFPR will not be responsible.
